Universal Behavioral Health Communication Form
Blue Cross and Blue Shield of Rhode Island, United Behavioral Health, Neighborhood Health Plan of Rhode Island and Beacon Health Strategies worked jointly to improve communication between the behavioral health community and the patients’ primary care physicians, by establishing a universal “Primary Care Provider Behavioral Health Communication Form”.
